A little questionnaire...
Many of your grades and addresses and other info are all mixed up in our database, so it'd be cool if you would take a minute to fill this out so we can get things straightened out.
If you didn't receive the cash I sent in the mail, you'll especially want to make sure I have your correct address. Also, if you are a sophomore and every time I see you, I shove you in a trashcan, it's probably because I have you marked down as a freshman in my records, so you should probably let me know.
Also...if you are new to Orchard Ave or you are just checking us out and would like to get on our (semi) monthly calendar mailer then feel free to fill this out.
